Candidates at MIP are able to offer reduced fees on a limited basis. In order to be considered for a significantly reduced fee (under $100/week), you must be able to commit to meeting at least 3x/week for a period of time. NOTE: This form must be completed by the patient, not a referring clinician.

Please note that MIP’s Referral Service will be temporarily pausing new referrals beginning September 19, 2026, for approximately six weeks.
